We’ve produced a new pocket guide in collaboration with Sue Ryder. Our improvement team worked with Sue Ryder’s care-at-home service in Arbroath to develop and test a new way of documenting the condition of clients at each home visit, to pick up on any deterioration and alert seniors so they can enable faster access to assessment and treatment.

We used the Model for Improvement approach to develop and design the tool. That started with mapping what happens during a home visit.  We identified six important areas we wanted the tool to cover. These were engagement, personal care, eating and drinking, going to the toilet, documentation and exit checks on leaving.

A team of Sue Ryder’s home carers tested the tool, making changes at each stage of development and clients and their families got involved in the testing too, giving valuable feedback.
